This form is a pdf file, so that it may be properly accessed and viewed on any platform. All that is neccessary is Adobe Acrobat, which is available for free download here.

After printing your order form, fill it out with your order, then mail it with your check or money order. Don't forget to include shipping and handling, as well as any applicable sales tax or other fees.

Shipping is $6.00 per order. Virginia residents please add 5% sales tax.

For foreign orders, please send a check drawn on a U.S. bank and include $15 for postage.
